Application process
Call, email, or visit office. Required documents: photo ID for all adults in household, social security cards for all household members, certificate of release or discharge from Active Duty (DD Form 214) showing discharge status or letter of service, proof of all household income (must be dated within 30 days), and proof of homelessness if currently unhoused. If currently housed: copies of current signed lease or landlord agreement and copy of eviction notice or court ordered move-out date, if applicable.

None for most services; vary for transitional housing
