Provides health care assessment and case management to Native Americans. Services include transportation to and from medical appointments, medication deliveries, home visits, exercise groups, eye exams and eye glasses, podiatry workshops, individual meetings and group workshops with a Registered Dietician Nutritionist, and disease prevention education. Also provides sobriety support groups (Native Sisters, White Bison, and AA), and QPR and ASIST suicide prevention trainings. Offers after school youth coalitions, monthly elderly luncheons, and annual community events. Provides food boxes, hygiene supplies, and diapers for adults and children.
