Must be age 22 and younger and live in Ash Fork, Bagdad, Chino Valley, Humboldt, Mayer, Prescott, Prescott Valley, and Seligman. Must bring child's original birth certificate and immunization records, parent/guardian's photo ID, and proof of address.
