Complete online application through the Human Services Department Client Portal or visit website for list of local Community Action Program offices. Photo identification, proof of U.S. Citizenship or Qualified Noncitizen Status, documentation of household income received for 30 days, and most recent gas, electric, and water (restrictions may apply) bills of current residence. Additional documentation may be requested by the CAP office to determine eligibility.
