Psychological Assessment | Solari Community Support Network Eligibility and Care Services
Evaluates assessment packets for both adults and children received by healthcare providers or AHCCCS providers to determine whether an individual meets diagnostic and functional criteria to receive comprehensive, community based mental health care if eligible for Serious Mental Illness (SMI) or Serious Emotional Disturbance (SED) services by following the states guidelines/criteria.

Application process
Call or visit website for information. Contact local T/RBHA or Crisis Preparation & Recovery to schedule an appointment. Behavioral health service providers complete an evaluation, then submit the SMI or SED Assessment Packet.
Eligibility
Adults and children who may have a Serious Mental Illness (SMI) or Serious Emotional Disturbance (SED). Must have a behavioral health service provider apply for the evaluation.
