Arizona youth in out-of-home placements who are involved with the Department of Child Safety. Plan is available regardless of enrollment in AHCCCS/Medicaid.

Application process

Call or visit website to locate a provider. Bring photo ID, AHCCCS card, Social Security card, list of medications, and name of family doctor to first appointment.
