Applicants must fill out the ADA Paratransit Eligibility Certification form. The process may involve contact with professional references and/or an in-person assessment of functional travel skills. This may take up to three weeks, as the office will confirm that applicants are unable to use the fixed-route bus system (Sun Tran) because their disability prevents them from doing so.
