Provides care for the terminally ill and their families to meet their physical, emotional and spiritual needs. Service provided in the person's home, assisted living facility, skilled nursing facility, or other facility by professional nurses, social workers, counselors and trained volunteers. Volunteers provide respite for caregivers. Nursing care focuses on pain control and symptom management. Offers various support groups.
Eligibility
People who have been diagnosed with terminal illnesses and life expectancy of 6 months or less, their families, bereaved, and caregivers for hospice services. People with chronic medical conditions who are not eligible for or on hospice services for palliative services.
