The teams travel to all parts of Maricopa County, providing care in parks, permanent supportive housing complexes, social services, community agencies and more.

Provides transitional housing and services to US Military active duty members and veterans. Services include a women's program; spiritual program; and navigation services to available services. Also facilitates annual Veterans Stand Down in Bullhead City and leads the Veterans Resource Team in the Kingman Regional Veterans Treatment Court. Provides in-home hospice care for adults with a terminal illness. Team includes physicians, nurses, social workers, bereavement counselors, chaplains, home health aides, therapists, and volunteers. Offers bereavement counseling for the family for one year following the death of their loved one.
