Provides medical respite for people experiencing homelessness who are discharged from a hospital after treatment of a serious illness or major surgery. Individuals do not need health insurance to receive services through medical respite program.

Eligibility

People ages 18 and older who are homeless or imminently homeless who are currently hospitalized or being treated in a hospital emergency room and have nowhere to go after being discharged and will need assistance with care after leaving the hospital. Must have acute or active medical needs that are likely to improve within 4-6 weeks, or needs end of life care. Must be behaviorally appropriate to live in group setting, adhere to facility rules, and participate in own care.
